Cerita Deus-Ex: HR sendiri mengambil waktu 25 tahun sebelum kejadian pada Deus-Ex pertama, yaitu pada tahun 2027. Di tahun ini manusia sudah sangat maju dengan nano-teknologi. Sehingga sudah wajar jika beberapa bagian tubuh manusia diganti dengan perangkat elektronik. Player mengambil peran sebagai Adam Jensen, seorang bodyguard swasta dimana perusahaan tempatnya bekerja mengkhususkan diri dalam pengembangan nano-teknologi. Suatu hari Adam melihat kasus penyerangan yang ditujukan terhadapperusahaan tempatnya bekerja. Namun yang aneh adalah Adam mendadak menjadi tersangka dalam penyerangan tersebut. Dari sinilah cerita Deus-Ex: HR dimulai.

Untuk gameplaynya sendiri mengambil genre FPS-RPG, namun ada beberapa bagian yang nantinya mengambil sudut pandang orang ke-3 (3rd person) juga. Disini player bisa meng-kustomisasi bagian-bagian tubuh Adam dengan perangkat-perangkat keras (hardware) yang ditemui. Sehingga nantinya tubuh Adam akan lebih hebat ketika menjalani misi yang ada.
